Hepatitis A Outbreak Among Men Who Have Sex with Men, Rouen, December 2016–April 2017
Introduction: Since February 2016, various European countries have reported the circulation of several specific strains of the hepatitis A virus (HAV) of genotype IA, primarily affecting men who have sex with men (MSM). This study describes the characteristics of the hepatitis A outbreak that has occurred in Normandy since late 2016 and its links to ongoing outbreaks in other European countries, as well as the control measures implemented. Methods: A description of the outbreak was conducted using data collected through mandatory reporting. Enhanced surveillance of reported hepatitis A cases was implemented using a standardized questionnaire attached to the mandatory reporting form, which collected data on risky sexual behaviors and modes of encounter. Results: Between December 19, 2016 (week 50 of 2016) and April 30, 2017 (week 17 of 2017), a total of 30 cases of hepatitis A residing in Normandy were reported to Santé publique France, of whom 24 were men (male-to-female ratio: 4; median age = 26 years); 20 cases resided in the Seine-Maritime department, and 17 male cases reported having sex with men. Among the 24 men, 21 epidemic cases were classified as “confirmed” (20 VRD_521_2016 strains and one RIVM HAV16-090 strain), 1 outbreak case was classified as “probable,” and 2 sporadic cases were identified during this period. Cases reported primarily meeting their sexual partners anonymously and in group settings. Dating sites and mobile apps were the primary means used to meet these partners. Conclusion: The ongoing hepatitis A outbreak in the Rouen metropolitan area was detected at a time when several European countries were reporting clusters of cases involving the same viral strain. A vaccination campaign targeting MSM in the Rouen metropolitan area was launched in April 2017, with the aim of controlling the outbreak.
