Impact of hypertensive disorders of pregnancy on the development of hypertension
Objective - Hypertensive disorders of pregnancy (HDP) are a major cause of maternal and fetal morbidity and mortality. The objective of this study was to estimate the impact of HDP on the development of chronic hypertension in women during the first few years following childbirth. Materials and Methods - This national cohort study was based on data from the National Health Data System (SNDS). We included all primiparous women without chronic hypertension (HTN) in the year preceding pregnancy who gave birth in France between 2010 and 2018. These women were followed for an average of 3.0 years starting from the 6th postpartum week. During follow-up, the onset of chronic HTN was identified by at least three prescriptions of antihypertensive medication. The association between DHG and the onset of HTN was assessed using crude and adjusted Cox models. Results - A total of 2,663,573 women were included with an average follow-up of 3.0 years. Among them, 180,063 (6.73%) developed a GHT, including 57,595 (2.13%) with preeclampsia (PE) and 113,803 (4.27%) with gestational hypertension (GH). The hazard ratios for developing hypertension, compared to women without DHG, were 6.03 (95% CI: [5.89–6.17]) for GH, 8.10 [7.88–8.33] for PE, 12.95 [12.29–13.65] for early PE, 9.90 [9.53–10.28] for severe PE, and 13.17 [12.74–13.60] for PE associated with GH. Duration of exposure was associated with an increased risk of hypertension for PE, but not for HG. Conclusion - DHG significantly increases the risk of developing hypertension in the years following childbirth.
