Hepatitis C Screening: Screening for Hepatitis C Before Endoscopy in the Hepatology and Gastroenterology Outpatient Clinic

Introduction – Routine serological screening for hepatitis C virus (HCV) once in a lifetime is recommended by the French Association for the Study of the Liver, but not by the French National Authority for Health. Screening focused on individuals over the age of 40 would appear more appropriate, as the prevalence of hepatitis C increases with age. Materials and Methods – The objective of this study was to evaluate the feasibility (number of serology tests offered) and acceptability (number of serology tests performed) of HCV screening prior to endoscopy among individuals aged 40 and older seen in gastroenterology clinics at non-university hospitals, and to determine whether the prevalence after age 40 is higher than that of the general population (0.86%). Results – As of June 1, 2023, 490 patients were enrolled across 8 different hospitals in 6 regions of mainland France; 97.4% of patients accepted the HCV serology test, and 97.6% of the prescribed tests were performed; 55.5% were men and 44.4% were women, with a mean age of 58 years (range 40–90). The HCV serology positivity rate was 6% (29 patients). No prior HCV serology was known. Risk exposures associated with a positive HCV serology result included drug use in 19 patients, a history of transfusion in 6 cases, and origin from an endemic country in 5 patients; 90% of positive serology results were in men, and the mean age was 65 years (range 49–85). The mean liver stiffness measurement was 8.7 kPa; 11 of the 28 patients tested had a positive HCV viral load and were treated. Conclusion – Routine screening for hepatitis C after age 40 and prior to upper gastrointestinal endoscopy is feasible, well accepted, and enables the management of a large number of patients.
