At the Toulouse Health Center, five health mediators work in two underserved neighborhoods: they serve as a bridge between and support for healthcare professionals, helping patients and empowering them to take control of their own health. The evaluation shows that patients gain the ability to access information, make it their own, and make informed decisions. And that the work of healthcare professionals is improved as a result.
