Investigation of a non-infectious outbreak at a nursing home in Barentin (Seine-Maritime). March 2010

On Monday, March 15, 2010, the manager of a real estate company in Barentin (Seine-Maritime) contacted the regional health authorities to seek guidance on managing a scabies outbreak in a group setting. The social housing complex in question consists of three buildings (Alpha, Beta, and Omega) containing 71 units (approximately 200 residents). This report comes at a time when the residence had undergone renovations in the preceding months. The Interregional Epidemiology Unit (CIRE) of the InVS and the Haute-Normandie Regional Health Agency (ARS) conducted an investigation focusing on three areas: clinical, epidemiological, and environmental. Conclusions: 1. Against a backdrop of stress linked to a lack of communication within the residence, the renovation work, and suspicion of an infectious disease, an outbreak of skin symptoms occurred; 2. Responses to the questionnaire were received from a total of 168 residents, 14 of whom met the case definition. Cases presented with general symptoms or symptoms primarily affecting the skin (macules, papules, and pruritus); 3. The proportion of cases was no higher among children than among adults; 4. Environmental investigations did not identify any sources of contamination; 5. All households in the residence consisting of single mothers raising their children were affected; 6. No statistically significant association was observed between the presence of construction work and the onset of symptoms; 7. Given the various contextual factors, the final diagnosis pointed toward a non-infectious collective outbreak. It would have been beneficial to initiate an investigation into psychosocial factors as soon as the report was received, in order to better characterize the contextual factors and describe the spread of symptoms. This would also have enabled the formulation of recommendations regarding collective care for the individuals, taking their social context into account. (R.A.)
