Mortality in a cohort of incident cases of Parkinson’s disease identified in medical-administrative databases

Parkinson's disease (PD) affects approximately 160,000 people in France, and this number is expected to rise in the coming decades. Patients with Parkinson's disease have an increased risk of death; however, mortality among these patients has been little studied in France. Using national health insurance reimbursement data, a cohort of incident cases of PD enrolled in the general health insurance scheme was established. A validated algorithm was used to identify incident cases between 2009 and 2015. Their survival was estimated using the Kaplan-Meier method. Their mortality rate was calculated, and their risk of mortality was compared to that of the general population, stratified by sex and age, using standardized mortality ratios and the Comparative Morbidity Figure (CMF). A total of 131,418 incident cases of MP, followed for an average of 3.5 years (standard deviation = 2), were included in our cohort. Among them, 31,670 died during follow-up, corresponding to an annual rate of 8.4 deaths per 100 person-years (9.2% among men, 7.3% among women). The 5-year survival probability for MP patients was over 60%, higher among women than among men (68% vs. 62%, respectively). Across all age groups, the risk of mortality among patients compared to the general population was twice as high; this risk was higher in women (HR=2.34; 95% CI: [2.21–2.47]) than in men (HR=2.15; 95% CI: [2.07–2.22]). Regardless of sex, this relative risk decreased with age (p<0.0001). These new data on Parkinson’s disease in France confirm the higher mortality risk among these patients compared to the general population and demonstrate the greater impact of the disease on mortality among younger patients and women.
