Maternal Deaths in France: Better Understanding for Better Prevention. 5th Report of the Confidential National Survey on Maternal Deaths (ENCMM), 2010–2012

For the 2010–2012 period, 256 maternal deaths were identified, representing 85 women who died each year in France from causes related to pregnancy, childbirth, or the postpartum period. This figure corresponds to a maternal mortality ratio (MMR) of 10.3 per 100,000 live births, which is stable compared to the previous period (2007–2009) and in line with the average for European countries. However, 56% of these deaths are considered “preventable” or “possibly preventable,” and in 60% of cases, the care provided was not optimal, indicating room for improvement. A key finding of this report is the one-third decrease in direct maternal mortality over the past 10 years, primarily due to the first statistically significant decline in mortality from obstetric hemorrhage, the incidence of which has been halved over 10 years. However, nearly all remaining deaths from hemorrhage are considered preventable, and this remains the leading cause of maternal mortality in France (11% of deaths). Certain disparities in maternal mortality remain unchanged and are a cause for serious concern. These include regional disparities: 1 in 7 maternal deaths occurs in the French overseas departments (DOMs), and the maternal mortality ratio in the DOMs is four times that of mainland France; and social disparities: the mortality rate among migrant women remains 2.5 times that of women born in France, with particularly high excess mortality among women born in sub-Saharan Africa, whose maternal mortality ratio is 3.5 times that of women born in France. Beyond the numbers, the authors identified 22 key messages from their analysis of all maternal deaths that occurred in France between 2010 and 2012. In line with the survey’s guiding principle, “better understanding for better prevention,” they focus on areas for improvement in care or its organization that are relevant to the preventability of these deaths and were repeatedly identified in this series of stories that are both unique and exemplary.
