Epidemiology of Strokes in France
Introduction – Nearly 10 years after the first National Stroke Plan (2010–2014), this study provides an overview of the epidemiology of strokes identified by the National Health Data System (SNDS), their management, and patient outcomes. Methods – Patients over the age of 18 hospitalized for a stroke in 2022 were identified using medical-administrative data, and analyses were stratified by stroke type. The prevalence of stroke as of January 1, 2023, was defined by the number of people alive on that date with a history of hospitalization for stroke or a long-term condition related to this disease (2012–2022). Incidence and prevalence rates per 100,000 inhabitants were calculated using population data from French national statistics. Patients were followed for up to one year after hospitalization for stroke to record, among other things, vital status, rates of admission to medical and rehabilitation care (SMR), post-stroke consultations with a neurologist, and medication use. Results – In 2022, 122,422 adults were hospitalized at least once for a stroke, and the prevalence of stroke was estimated at 1,086,795 cases. The overseas departments and regions (DROM), the departments of Côtes-d’Armor, Nord, Pas-de-Calais, Seine-Saint-Denis, several departments in the Southwest, as well as individuals residing in the most disadvantaged municipalities in mainland France, had rates significantly higher than the national average. Admission to a neurovascular intensive care unit was observed in 46.8% of patients, with significant variations by department, age, and sex. Among 6-month survivors, follow-up revealed a rate of admission to a stroke unit of 34.3% for ischemic strokes and 41.7% for hemorrhagic strokes, and only 28.8% and 18.8% of ischemic and hemorrhagic strokes, respectively, had been seen by a neurologist, and 19.5% of ischemic strokes were seen by a cardiologist within 6 months. One-year mortality was 20.8% among patients hospitalized for an ischemic stroke and 37.9% among those hospitalized for a hemorrhagic stroke. Conclusion – Significant variations in the incidence and management of strokes have been observed both geographically and over time since the 2010–2014 Stroke Plan in France. A very small proportion of patients currently undergo mechanical thrombectomy for ischemic strokes. These results highlight the need for more effective stroke prevention in certain regions and populations, for regular public awareness campaigns on the signs and symptoms of stroke and what to do in such cases, and for improving the availability and accessibility of neurovascular units (NVUs) and follow-up care for patients.
