My Preventive Health Checkup is a new, free appointment with a doctor, pharmacist, nurse, midwife, or physical therapist to discuss your health and receive personalized preventive care advice. You can participate if you’re between the ages of 18–25, 45–50, 60–65, or 70–75.

An educational brochure provides guidelines for discussing “My Prevention Assessment” with the people you support and helping them access the program.
A poster is available for reception centers, shelters, and waiting rooms.
Two informational resources (an introductory flyer and an educational video) are available for people in vulnerable situations and can be shared with the public. These resources are written in simple language, taking health literacy into account. They are also available in several languages (see documents and videos above).
