Distribution of genovars of urogenital Chlamydia trachomatis strains in mainland France and overseas territories in 2017–2018

Introduction - The National Reference Center for Bacterial Sexually Transmitted Infections (STIs) has undertaken a comparative study of the genovars circulating in Chlamydia trachomatis urogenital infections in metropolitan France and in the overseas territories and regions (DROM). Methods - Two prospective collections of urogenital samples positive for C. trachomatis were organized, one in 2017 (week 46) in mainland France and the other in 2018 (weeks 38 to 42) in the DROM, accompanied by the collection of demographic, clinical, and behavioral data. The samples were genotyped by sequencing the omp1 gene to determine the genovars. Results - Of the 1,861 samples collected, 1,353 (72.7%) could be genotyped, 772 from mainland France and 581 from the DROMs. All genovars from D to K are present in mainland France and the DROMs, with a predominance of genovar E (n=591, or 43.7%). The distribution of genovars is essentially the same across all geographic areas studied, although genovar G strains (n=15; 29.4%; p<0.05) in Polynesia and genovar J/Ja (n=21; 27.3%; p<0.05) in New Caledonia, which is statistically higher than in the other areas. A notable finding is the presence of genovar A strains (n=21; 5.8%), specific to trachoma, in the Indian Ocean. Conclusions - The distribution of genovars in metropolitan France and its overseas departments and regions is similar to that described in other countries worldwide, with a strong predominance of genovars E, F, G, and D. The presence of genovar A in urogenital infections in the Indian Ocean is the most striking finding of this study.
