Outbreaks of acute respiratory infections (ARI) in long-term care facilities in France, 2006–2007.
Introduction: Lower respiratory tract infections (LRTI) are the leading cause of infectious mortality in nursing homes for the elderly (Ehpad). The primary objective of reporting LRTI outbreaks in elderly care facilities, as formalized by a circular dated November 2006, is to reduce the morbidity and mortality associated with these outbreaks. The objective of the annual summary of these reports is to identify the characteristics of these outbreaks in order to adapt recommendations. Method: The report presented is based on reports submitted to the InVS by the Ddass, the coordination centers for the control of nosocomial infections, or the Regional Epidemiology Units (Cire) between August 2006 and July 2007. Since January 2007, a web-based application has facilitated real-time, interactive exchange of epidemiological information between DDASS, CIRE, and InVS. Results: During the 2006–2007 season, 64 outbreaks of acute respiratory infections (ARI) occurring in long-term care facilities for the elderly were reported to InVS. An influenza virus was identified in more than 30% of these outbreaks. A total of 41 outbreaks were reported in nursing homes, 10 in retirement homes, 9 in long-term care units, and 4 in other hospital departments. The average attack rates were 22% for residents and 7% for staff members. The average case fatality rate among residents was 4%. Average influenza vaccination coverage was 91% for residents and 38% for staff. Staff members were affected in 51% of the outbreaks and in at least 4 outbreaks before residents were affected. The average duration of an outbreak is 13 days. Control measures are implemented within an average of 7 days and in 36% of cases after the outbreak is reported. When control measures are implemented late, the average duration of the outbreak is longer, with residents facing an increased risk of becoming ill.
