Unexplained health issues among schoolchildren in Bert, Allier: April 14 – July 4, 2014

On April 14, 2014, the Allier Prefecture reported to the Auvergne Regional Health Agency (ARS) that five children attending the elementary school in Bert, a town of 250 residents, were exhibiting “allergy-like” symptoms (sneezing, coughing, and a scratchy throat). These symptoms recurred repeatedly among a varying number of students in the same class over a period of more than two months. Numerous stakeholders became involved in the matter to manage the various episodes—often on an emergency basis—and to understand their origin. The search for physical environmental exposure was initially prioritized, but the epidemiological and environmental investigations conducted did not find any objective evidence supporting exposure to a toxic substance. The investigation was gradually expanded to include other factors—psychological and psychosocial—as recommended by the Santé publique France guide Diagnosis and Management of Unexplained Collective Syndromes. This report describes the various stages in managing these events, the challenges of coordinating the many stakeholders, and the sensitive communication surrounding such events in school settings.
