The care pathway for people living with diabetes in Réunion, eight years prior to their amputation in 2021

Introduction – The prevalence of diabetes and its foot-related complications is particularly high in Réunion. Just under 400 lower-limb amputations (LLAs) were recorded in 2023, with a hospitalization rate 1.5 times higher than the national average. This complication is a major concern for healthcare providers. A study was conducted to describe the characteristics of people living with diabetes who have undergone amputation and their pre-amputation care pathways.Methods – A descriptive analysis using data from the National Health Data System (SNDS) was conducted over an eight-year period (2014 to 2021) to trace the pre-amputation care pathways of 230 people living with diabetes who underwent amputation in 2021.Results – The study highlighted the high vulnerability of people who have undergone amputation, with complex profiles (30% with a history of amputation, 70% with at least one risk factor), and actual care pathways that deviate significantly from recommendations: inconsistent medication regimens, insufficient use of healthcare professionals, and inadequate adherence to recommended medical and laboratory tests.Conclusion – This regional care pathway study underscores the importance of improving the care pathway for people living with diabetes and preventing gaps in care. The results will enable the development of recommendations and an action plan for prevention and management, in coordination with the Réunion Nutrition and Diabetes and Obesity Prevention Program (PRNDO) and the 2024 Diabetic Foot Guidelines of the Francophone Diabetes Society (SFD).
