Telephone Smoking Cessation Helplines: What Organizational Structure Ensures Effectiveness? A Literature Review

Introduction - The Tabac info service telephone helpline, established in 1998, is now a fully-fledged smoking cessation support service, organized in accordance with evidence from the literature. The objective of this study is to analyze recent literature on the effectiveness of telephone-based smoking cessation support services (known as quitlines). Methods - A literature review was conducted in January 2022 via PubMed, supplemented by a search of gray literature and a telephone conversation with the managers of U.S. quitlines. Results - Quitlines are effective in helping smokers who have received multiple counseling sessions, whether the calls are incoming (RR=1.38) or outgoing (RR=1.25); smoking cessation rates are higher among individuals who received 3 to 5 calls versus 1 call. The North American Quitline Consortium (NAQC) recommends offering at least a 2-week starter kit of nicotine replacement therapy (NRT), a measure that has proven effective in the United States; furthermore, motivational interviewing may be effective only if used at high intensity (2 to 11 sessions). Tobacco control measures all have a positive impact on quitline utilization; a study of campaigns conducted in the United Kingdom showed that positive-toned communication campaigns are the most effective at increasing quitline utilization rates. Conclusion - Three implications emerge for the 39 89 service: increasing usage through communication campaigns, facilitating access to TNS via teleprescription, and continuing the outbound call strategy.
