Smoking Cessation in Metropolitan France: Recent Trends and Associated Factors Based on Data from the 2021 Santé publique France Barometer

Introduction – In France, tobacco use is the leading cause of preventable death and is associated with significant social inequalities. Nearly two-thirds of smokers report wanting to quit, but few succeed. The objective of this article is to examine smoking cessation over the 2016–2019 period, marked by a sharp decline in smoking rates. Methods – This study is based on data from the 2021 Santé publique France Health Barometer, a telephone survey conducted on a random sample of the population residing in mainland France aged 18 to 85. The analysis focuses on the 24,446 respondents whose smoking status was reported. Factors associated with smoking cessation during the 2016–2019 period, compared to daily smoking in 2021, were estimated using logistic regressions. Results – In 2021, 23.9% of adults were former daily smokers, and 29.8% of these had quit smoking within the past five years, proportions that have increased since 2010. Among both men and women, quitting smoking between 2016 and 2019 was associated with being older, having a higher level of education than a high school diploma, and daily use of e-cigarettes. The likelihood of having quit smoking was lower among the unemployed, people with a perceived difficult financial situation, those exceeding low-risk alcohol consumption guidelines, and, among women, those who had experienced a major depressive episode in the past year. Conclusion – This study highlights the persistence of social inequalities regarding smoking cessation and the need to adapt public policies accordingly. Furthermore, among the factors associated with smoking cessation, additional research is needed to better understand the complex interactions between tobacco and alcohol use and mental health.
