Outbreak of foodborne strep throat at summer camps in Corsica, July 2012

An outbreak of streptococcal pharyngitis (SGA) among children and adolescents at a summer camp was investigated at a campground in Haute-Corse in July 2012. The majority of cases were identified within 48 hours, raising suspicion that the outbreak was caused by food served at the camp, although this mode of transmission is rare. The objective of the retrospective cohort investigation was to confirm this hypothesis and identify the source of the infection. A clinical case was defined as any person who, between July 18 and 31, presented with at least one sore throat, tonsillitis, or pharyngitis, associated with a positive rapid diagnostic test (RDT) or one of the following signs: fever, headache, or dysphagia. Cases classified as primary cases linked to food exposure were those occurring before July 25; later cases were considered secondary cases linked to human-to-human transmission. The epidemiological investigation conducted at the summer camps identified 156 cases of streptococcal tonsillitis, 46 of which were confirmed by RDT and/or throat swab. The attack rate was 30.3% for primary cases and 2.4% for secondary cases. It was higher among those on full board (38.5%) than among those on half board (13%). The meals posing the highest risk were served at noon on July 20 and 21, but it was not possible to determine with certainty the food item(s) responsible for the contamination. The investigation of service and kitchen staff revealed that the first cases of strep throat had appeared on July 18 and 19 among servers, who were likely the source of the contamination. The investigation also identified 2 asymptomatic carriers of group A streptococcus (GAS) and 8 patients. The GAS strain involved, a rare emm4.4 type, was identical among the cooks, children, and activity leaders, highlighting a common source for this outbreak.
