Surveillance of Outbreaks of Acute Respiratory Infections and Acute Gastroenteritis in Long-Term Care Facilities for the Elderly in the Pays de la Loire Region. Surveillance Results for the Winter of 2010–2011. Evaluation of the Surveillance System
A system for monitoring and reporting clusters of acute respiratory infections (ARI) and acute gastroenteritis (AGE) was proposed in 2010 to the 557 nursing homes (Ehpad) in the Pays de la Loire region. Simple tools for self-monitoring and outbreak management were selected by a regional working group; their distribution was accompanied by training provided by the region’s public health networks. To assess the perception and use of the system, an evaluation survey was conducted after the winter season, in April 2011. During the winter of 2010–2011, 179 clusters of cases (52 episodes of acute respiratory infections and 127 of gastrointestinal infections) were reported. The completeness rate of these reports was estimated at 75%. The average attack rates among residents were 18% for ARIs (685 cases) and 28% for ARI (2,527 cases). The attack rates for ARI and ARIs were lower in facilities that implemented control measures early (within the first three days). This surveillance was deemed very useful by the facilities and contributed to the development of a culture of infection risk management in nursing homes. The surveillance and evaluation data helped identify strengths to be consolidated (training, local organization of surveillance) and areas for improvement, particularly the simplification of tools.
