Proportion of carpal tunnel syndrome cases attributable to occupational factors among high-risk occupations and industries in two French departments
Introduction - In France, the underreporting rate for carpal tunnel syndrome (CTS)—the second most commonly recognized occupational disease—was estimated at 43% in 2015. The objective was to determine the attributable risk fractions (ARFs) for CTS among exposed individuals, within high-risk occupations and sectors. Materials and Methods - Data on patients who underwent surgery for CTS from two studies conducted in Maine-et-Loire (2002–2003) and in Bouches-du-Rhône (2008–2009) were compiled to calculate the FRAE for occupations or sectors of activity (employment at the time of surgery) among patients exposed to them. The calculation was based on determining the age-adjusted relative risk of the occupation or sector using the Mantel-Haenszel method. Results - Among male and female manual workers, three-quarters of SCC cases were attributable to the excess risk associated with occupational exposure. The sectors of activity with the highest SCC FRAEs were the leather and footwear industry for men (FRAE=93%) and the automotive industry for women (FRAE=89%). For specific occupations, these were skilled roofers among men (work-attributable risk fraction = 97%) and female vegetable and horticultural workers among women (work-attributable risk fraction = 94%). Within the same sector of activity, certain occupations had higher work-attributable risk fractions than others, including in sectors that did not overall present an excess risk of occupational diseases. Discussion - Calculating FRAEs for different sectors of activity and/or SCC occupations allows, when such a calculation is possible, for the identification of high-risk jobs and thus contributes to targeting these jobs for workplace prevention initiatives, complementing occupational disease statistics.
