Prevalence of HIV infection in a sample of men who have sex with men in Paris. The 2009 Prévagay Survey (ANRS-InVS*).

Introduction. National estimates of HIV incidence in France indicate that men who have sex with men (MSM) represent the most affected population, accounting for nearly half of all new infections each year. The objective of this study was to estimate the HIV incidence rate in a sample of MSM frequenting gay social venues in Paris. Method. In 2009, the Prévagay study collected behavioral questionnaires and blood samples from a sample of men frequenting Parisian bars, saunas, and backrooms. The biological samples were tested for HIV infection. Among infected subjects, the EIA-RI test was used to indicate recent infection. To rule out false-positive results for recent infection due to treatment, we tested the samples for the presence of antiretrovirals. Results. Among 886 participants, 157 (18%) tested positive for HIV. The overall incidence rate is estimated at 3.8 per 100 person-years (PY) [95% CI: 1.5–6.2]. The incidence rate is estimated at 3.5 per 100 PY [0.1–6.1] among the 557 men who had tested negative for HIV in the previous year, and at 4.8 per 100 PY [0.1–10.6] among those (n = 329) who had never been tested before or had not been tested for more than a year (the difference being non-significant). Conclusion. This study is the first to estimate an HIV incidence rate among MSM attending community-based facilities. The rates obtained indicate a high level of transmission of the infection among sexually active individuals, despite a high level of testing uptake. These results confirm the need for effective prevention programs targeting MSM with risk behaviors.
