Prevalence of gastrointestinal carriage of glycopeptide-resistant enterococci (GRE) in healthcare facilities in the Lorraine region, June 2007
In January 2007, the Nancy University Hospital reported an outbreak of infections and colonizations caused by glycopeptide-resistant enterococci (GRE). In April, the increase in reports of GRE infections and colonizations from healthcare facilities in Lorraine suggested that the outbreak had spread throughout the region. To assess the prevalence of ERG carriage in the gastrointestinal tract and raise awareness among healthcare facilities in Lorraine about this emerging issue, a prevalence survey was conducted by the Eastern Interregional Epidemiology Unit and the Eastern Coordination Center for the Control of Nosocomial Infections in June 2007. A rectal swab was offered to all patients present on a given day in high-risk units (hemodialysis, hematology-oncology, surgery, intensive care, geriatrics) at the 26 healthcare facilities that had admitted, via transfer from the University Hospital, patients carrying (infected or colonized with) ERG. The isolated ERG strains were sent to the University Hospital’s bacteriology laboratory for confirmation and typing. Among the 2,718 patients included in the study, 48 (1.8%) were ERG carriers, of whom 31 (65%) were previously unknown. The prevalence ranged from 0% to 11.5% depending on the healthcare facility. The survey enabled 10 facilities (40%) to identify one or more (up to 13) new ERG carriers. The results of this survey confirm the spread of ERG in the region’s healthcare facilities, and molecular analyses of the strains suggest they belong to the same epidemic clone. The prevalence (1.8%) observed in this survey, significantly higher than the results of the 2006 Onerba-CNR-InVS survey (0.3%), underscores the importance of a regional policy to control the emergence of ERG in Lorraine.
