Signs of Influenza A(H3N2) and BNationally, despite an increase in SOS Médecins activity related to influenza since early January, the Sentinelles network has not observed the epidemic threshold being exceeded for the week of January 5 (2015-02). However, the increase in consultations with sentinel physicians for flu-like symptoms suggests that an epidemic may begin this week (03) or next week (04).In Bourgogne/Franche-Comté, the flu has been circulating since early December according to the virology laboratory at the Dijon University Hospital (which receives samples from Grog sentinel sites and hospitals): the percentage of samples testing positive for an influenza virus rose from 4% (2 samples), 11% (7) to 15% (12) for weeks 2014-52, 2015-01, and 2015-02; these were type A strains, except for last week (02) when 3 type B strains were detected for the first time among the 12 positive strains.Since early January, influenza diagnostic activity has been increasing in our emergency departments and has exceeded the epidemic threshold for the past two weeks across our three SOS networks, particularly in Sens, where clinical influenza accounts for one in seven diagnoses since January 1 (Figures 1 and 2). We are also observing an increase in outbreaks of acute respiratory infections (ARI) in nursing homes (Figure 8), including 4 with influenza A diagnoses, as well as a growing number of severe influenza cases admitted to intensive care. The number of sentinel physicians in Burgundy and Franche-Comté (< 10) is insufficient to allow for local interpretation. However, a regional pilot project is underway involving the sentinel networks of Burgundy and Franche-Comté, virologists at the Dijon University Hospital, the Regional Union of Health Professionals (URPS), the Cire, and the Burgundy Regional Health Agency (ARS): the aim is to establish surveillance of respiratory infections, and even gastroenteritis, in outpatient settings, bringing together primary care providers wishing to participate in a local network, and aligning with the surveillance objectives of the InVS. The 30 Grog sentinel sites saw an increase in activity in late December and again since early January. They have observed a recent increase in acute respiratory infections, particularly clinical influenza cases, which accounted for 12% of consultations last week (02). These local observations indicate that seasonal influenza is currently affecting Burgundy and Franche-Comté, and suggest a peak in activity in February.
