Study of the profile of participants in the VIHTest program, which offers free, prescription-free HIV testing in five French regions

Introduction – Since January 1, 2022, free human immunodeficiency virus (HIV) testing without a prescription has been available at all private laboratories through the VIHTest program. This service has been rolled out nationwide, following the results of the "Au labo sans ordo" (Also) pilot program. A survey by Santé publique France was conducted in five regions to monitor the rollout of this program. Methods – A cross-sectional survey was conducted from September 15, 2023, to March 15, 2024, at a selection of participating laboratories in Grand Est, Normandy, Occitanie, Guadeloupe, and Martinique. Laboratories offered an anonymous online self-administered questionnaire to individuals over the age of 18 with social security coverage who were undergoing HIV testing. The profile of VIHTest users was compared to that of individuals tested via prescription using multivariate logistic regression, adjusted for gender, sexual orientation, age, and variables with a p-value ≤0.2 in univariate analysis. Results were analyzed separately for each region. Results – A total of 6,531 questionnaires were analyzed, including 2,004 (30.7%) prescription-based tests and 4,257 (69.3%) VIHTest. Compared to the population tested via prescription, the VIHTest service attracted an older population. The remaining characteristics associated with the VIHTest program varied by region. For example, in Occitanie and Normandy, VIHTest users had fewer prior HIV tests and fewer sexual risk factors than users of prescription tests. Conclusion – Our survey showed that the profile of VIHTest users varies from one region to another and differs from the profile of users in the Also pilot program. It provided other interesting insights into the rollout of the service. The heterogeneity of results across regions supports and reinforces the need for local management of the service. It also highlighted the central role played by laboratories in the promotion and rollout of VIHTest. These results open up interesting avenues for further consideration, particularly with the expansion of the program to other STIs for those under 26.
