Prevalence of self-reported high blood pressure in France’s overseas departments and regions: the 2021 Public Health France Barometer survey
Background – Hypertension is the leading risk factor for stroke and a major risk factor for cardiovascular morbidity and mortality. The objective of our study was to estimate the prevalence of reported HTN in Guadeloupe, Martinique, French Guiana, and Réunion and to describe several indicators of awareness and management. Methods – Data from the 2021 Santé publique France DROM Health Barometer were used. More than 6,500 people (1,511 in Guadeloupe, 1,526 in Martinique, 1,478 in French Guiana, and 2,004 in Réunion), aged 18 to 85 and residing in four overseas departments and regions (DROM), were interviewed by telephone about hypertension. The analyses were weighted to account for the sampling plan and response rate to ensure they were representative of the population of each DROM. Data from mainland France used for comparison were drawn from the 2019 France Public Health Barometer. Results – The prevalence of self-reported hypertension was 31.5% in Martinique, 29.9% in Guadeloupe, 22.7% in French Guiana, and 20.8% in Réunion. The prevalence was significantly higher among women than among men in each DROM. The proportion of adults self-reporting hypertension who were treated with antihypertensive medication did not vary by region or by sex and exceeded 80%, as in mainland France. Between 65% and 73% of adults with hypertension reported having received advice on lifestyle changes in the DROMs, compared to 58.5% in mainland France. In French Guiana, 51.5% of hypertensive individuals owned a home blood pressure monitor, compared to 53.8% in Réunion and over 70% in Guadeloupe and Martinique. Conclusion – The prevalence of hypertension remains very high in the DROMs, particularly among women. However, several aspects of care did not differ from mainland France, with several favorable indicators such as the provision of lifestyle and dietary advice or the ownership of home blood pressure monitors. Since complications of hypertension are common in the DROMs, nutritional prevention must be strengthened, particularly among women.
