Evaluation of the West Nile virus surveillance system in Guadeloupe using the Oasis method
Introduction - Surveillance for West Nile virus (WNV)—a vector-borne zoonosis—in Guadeloupe was established following the virus’s first detection in 2002, with the aim of ensuring early warning and rapidly implementing appropriate control measures. For years, it was organized on a sector-by-sector basis, with components for human, animal, and entomological surveillance. Since 2020, stakeholders in these areas have developed a more collaborative and transdisciplinary integrated strategy aimed at improving the system’s operation and performance, and have recommended evaluating it regularly. Method - In 2023, the West Nile virus surveillance system in Guadeloupe was evaluated using the Oasis method (Tool for Analyzing Health Information Systems) to redesign the system in collaboration with its stakeholders. Semi-structured interviews were conducted in Guadeloupe and at the national level. A collective scoring session was organized; the results and recommendations were shared with all participants. They were finally discussed during a workshop to collectively define an action plan. Results - The system’s main strengths (diagnostic laboratories, surveillance tools, stability) and weaknesses (coordination, information sharing, specificity) were used to formulate and prioritize four recommendations: (i) publish a new interministerial guide for West Nile virus surveillance, (ii) establish a steering and coordination structure for the surveillance system in Guadeloupe, (iii) intensify awareness-raising among field actors, and (iv) create an annual epidemiological bulletin. Conclusion - The implementation of the recommendations, scheduled to begin in 2024, is being monitored by surveillance stakeholders. This work has revitalized the network and strengthened ties among stakeholders and could serve as a model for the surveillance of other vector-borne or zoonotic diseases of interest, such as leptospirosis.
