Mortality from valvular heart disease in France: patient characteristics and trends, 2000–2014

Introduction - In France, the incidence of hospitalizations for valvular heart disease is on the rise. The objective of this study was to estimate mortality due to valvular heart disease in France and to describe its trends between 2000 and 2014. Methods - Patients residing in France whose death certificates mentioned valvular heart disease were identified using the national CépiDC database (multicausal selection of death certificates) for the years 2000 to 2014. The demographic characteristics of the patients were described for the year 2014. Age-standardized national and departmental mortality rates, as well as national trends between 2000 and 2014, were analyzed. Results - In 2014, 11,179 death certificates mentioned valvular heart disease, with 56% listing it as the primary cause of death. The age-standardized mortality rates were 11.8 per 100,000 person-years (PY) for non-rheumatic valvular heart disease and 0.056 per 100,000 PY for congenital valvular heart disease, remaining stable between 2000 and 2014. In contrast, the standardized mortality rate for rheumatic valvular heart disease decreased by 42% between 2000 and 2014, standing at 3.5 per 100,000 PA in 2014. Regional disparities were observed for these conditions, with higher mortality from rheumatic valvular heart disease in eastern France and higher mortality from non-rheumatic valvular heart disease in the northwest. Non-rheumatic aortic stenosis was the most common valvular heart disease, with a standardized mortality rate of 10.3 per 100,000 population in 2014. Conclusion - Despite the aging population, improvements in the management of valvular heart disease and its risk factors may explain the stability in mortality from non-rheumatic valvular heart disease. The decrease in mortality from rheumatic valvular heart disease is likely due to improved management of acute rheumatic fever.
