Maternal Deaths in France: Better Understanding for Better Prevention. 6th Report of the National Confidential Survey on Maternal Deaths (ENCMM) 2013–2015.

For the period 2013–2015, 262 maternal deaths were recorded, representing one death every four days in France due to a cause related to pregnancy, childbirth, or their aftermath. This figure corresponds to a maternal mortality ratio (MMR) of 10.8 deaths per 100,000 live births over a one-year period, and an MMR of 8.1 deaths per 100,000 live births within the first 42 days, which remained stable compared to the previous period (2010–2012) and is in line with the average for European countries. However, 58% of these deaths are considered “preventable” or “possibly preventable,” and in 66% of cases the care provided was not optimal, indicating room for improvement. Certain inequalities in maternal mortality remain unchanged, including regional disparities —women living in the French overseas departments face a fourfold higher risk of maternal mortality compared to those in mainland France—and social disparities—the mortality rate among migrant women is higher than that of women born in France, with particularly high excess mortality among women born in sub-Saharan Africa, whose risk is 2.5 times that of women born in France. A striking finding of this report is the prominent role of cardiovascular diseases and suicides, the two leading causes of maternal mortality, each accounting for approximately one death per month. Another important finding is the continued decline in mortality from obstetric hemorrhage, which has been halved over 15 years, particularly due to a decrease in fatal hemorrhages caused by uterine atony. Beyond the numbers, the authors identified 30 key messages from their analysis of all maternal deaths occurring in France between 2013 and 2015. In line with the survey’s guiding principle, “better understanding for better prevention,” they focus on areas for improvement in care or its organization that are relevant to the preventability of these deaths and were repeatedly identified in this series of stories that are both unique and exemplary.
