In-hospital cardiac rehabilitation following myocardial infarction in France: insights from the PMSI-SSR
Introduction: This study examines hospitalizations for cardiac rehabilitation (CR) following a hospitalized myocardial infarction (MI). Methods: Data were extracted from the national databases of acute care hospital discharge summaries (PMSI-MCO) and post-acute care and rehabilitation (PMSI-SSR). First, hospital stays from the first half of 2011 with a primary diagnosis of MI were selected from the PMSI-MCO database. Only the first hospital stays of patients who were still alive were retained and then linked to the PMSI-SSR database for 2011. The primary purpose of care allowed us to distinguish patients in RC from those hospitalized in SSR for another reason. Logistic regressions were performed to compare regional RC rates. Results: Following an MI, one-third of patients were hospitalized in SSR: 22.7% for RC and 10.0% for an “other” purpose. The proportion of patients in long-term care decreased with age (from 40.6% among those aged 18–45 to 2.3% among those aged 85 or older) and was lower for women than for men (age-standardized rates: 18.8% vs. 23.9%); regional disparities were marked. Discussion-conclusion: These results pertain to inpatient RC, which constitutes the general framework for RC in France. They likely underestimate the actual figures somewhat due, on the one hand, to residual care in the outpatient sector and, on the other hand, to the likely suboptimal quality of coding in the PMSI-SSR database. However, they reveal notable differences across age groups, genders, and regions. (R.A.)
