Regional disparities in potential years of life lost prematurely due to cardiovascular disease in France (2013–2015)

Introduction - Cardiovascular diseases (CVD) are the second leading cause of death in France and the third leading cause of premature death. The objective of our study was to describe regional disparities in premature mortality for all CVDs, and more specifically for stroke and myocardial infarction (MI), in France, taking into account age at death. Method - Mortality data were extracted from the national database of medical causes of death (CépiDc-Inserm). Various age-standardized indicators were estimated at the national level in 2015: total mortality, premature mortality (deaths occurring before age 65), and potential years of life lost prematurely (PYLL). The PYLL were then broken down at the departmental level for deaths occurring between 2013 and 2015. Results - In 2013–2015, in France, premature mortality from CVD accounted for 8.9% of overall CVD mortality. The average number of PYLL per CVD was 10.4 years per person. Significant regional disparities were observed across the country. Overall, the northern and northeastern regions of mainland France, as well as the overseas departments and regions (DROM), had higher APVPp rates than the national average. In contrast, several departments in the Île-de-France and Auvergne-Rhône-Alpes regions had APVPp rates lower than the national average. Discussion - Significant regional disparities in premature mortality from CVD persist across France and correlate with the distribution of modifiable cardiovascular risk factors. Taking age at death into account allows for a more accurate estimation of the cardiovascular burden associated with mortality at the departmental level. Continuing efforts to screen for and prevent these risk factors appears to be a priority in departments with excess APVPp and could help reduce the preventable portion of mortality associated with these conditions.
