SurCeGIDD Surveillance: Screening and Diagnosis of HIV, Hepatitis B and C, and Bacterial STIs in CeGIDD in 2020

Introduction - The CeGIDDs (Free Centers for Information, Screening, and Diagnosis of Human Immunodeficiency Virus (HIV) Infections, Viral Hepatitis, and Sexually Transmitted Infections (STIs)) were established in 2016. This article describes, for the year 2020, the characteristics of clients as well as the screening and diagnostic activities carried out in these facilities, comparing them to 2018. Method - This is a repeated cross-sectional study based on surveillance data continuously collected by the SurCeGIDD system, which relies on the secure transmission of individual data regarding patients in a predefined format. The sociodemographic and behavioral characteristics of the patients, as well as their reasons for seeking care, were described. The proportions of patients tested and the positivity rates for HIV, hepatitis B (HBV) and C (HCV), syphilis, Neisseria gonorrhoeae (NG), Chlamydia trachomatis (CT), and Mycoplasma genitalium (MG) were analyzed by partner sex. Results - In 2020, 336,333 consultations were reported by 50.3% of the 336 CeGIDDs surveyed. Visits to CeGIDDs declined sharply in the second quarter of 2020 (-58% compared to the first quarter). In 2020, the majority of patients were men (62.1%) and young people under 30 (64.5%). About a quarter (23.5%) of clients were born abroad. The proportion of men who have sex with men (14.8%) and transgender people (0.38%) increased compared to 2018. The most common reasons for seeking care remained exposure to risk (60.4%) and routine screening (43.3%). Certain reasons for seeking care increased between 2018 and 2020, such as starting or continuing pre-exposure prophylaxis. An increase in the positivity rate between 2018 and 2020 was observed for HIV (from 0.37% to 0.41%), but especially for NG (from 2.8% to 4.0%), likely due to changes in the characteristics of the patients seen. At the same time, rates decreased for HBV (from 1.3% to 0.93%) and HCV (from 0.94% to 0.60%). They remained relatively stable for CT (7.0% in 2020), MG (6.8%), and syphilis (1.0%). Conclusion - Despite a decline in consultations in 2020 due to the COVID-19 pandemic, the CeGIDDs continued to fulfill their screening and diagnostic missions as part of a comprehensive approach to sexual health. The SurCeGIDD surveillance system provides data for monitoring the national sexual health strategy; therefore, it is essential to improve the comprehensiveness and completeness of the collected data.
