Screening and diagnosis of HIV, hepatitis B and C, and bacterial STIs at CeGIDD in 2018: individual data from the SurCeGIDD surveillance system

Introduction - The CeGIDDs (free centers for information, screening, and diagnosis of human immunodeficiency virus (HIV) infections, viral hepatitis, and bacterial sexually transmitted infections (STIs)) were established in 2016. This article analyzes their screening activities based on the SurCeGIDD surveillance system. Method - The SurCeGIDD surveillance system relies on the secure transmission of individual data regarding patients, in a predefined format. Sociodemographic data, testing for HIV, hepatitis B and C, syphilis, Neisseria gonorrhoeae (NG), Chlamydia trachomatis (CT), and Mycoplasma genitalium (MG), diagnoses, and sexual behaviors were analyzed. The proportions of clients tested and positivity rates were described for each pathogen and by type of client population. Results - In 2018, 382,890 consultations were reported by 44.6% (n=143) of the CeGIDDs, based on 317 CeGIDDs identified in 2016. The majority of patients (59.8%) were men, 40.0% were women, and 0.2% were transgender. Positivity rates were 0.37% for HIV, 1.31% for HBV, 0.94% for HCV, 7.16% for CT, 2.83% for NG, 1.04% for syphilis, and 5.96% for MG. Depending on the pathogen, positivity rates were higher among men who have sex with men, foreign-born individuals, transgender people, and patients in the French overseas departments and in Île-de-France. Conclusion - Despite room for improvement in coverage and comprehensiveness, SurCeGIDD offers new insights into the vulnerability of the populations served. Its contextualized analysis will help tailor the national sexual health strategy to specific regions and populations.
