Vaccination coverage against human papillomavirus infections among girls aged 15 to 18 and factors influencing vaccination, France, 2021

Introduction - Our study aims to estimate HPV vaccination coverage among girls aged 15 to 18 in France, to explore the factors associated with this vaccination, and to describe the reasons for non-vaccination in metropolitan France. Method - The study is based on data collected during the 2021 Baromètre Santé survey. Participants were selected through random generation of landline and mobile phone numbers and interviewed between February and December 2021. Parents of girls aged 15 to 18 were surveyed about their daughter(s)’ HPV vaccination. The determinants of HPV vaccination were studied using univariate and multivariate Poisson regression. Analyses were performed using Stata® SE 15.1 (StataCorp, USA). Results - HPV vaccination coverage is estimated at 43.6% [40.1–47.1] in mainland France, 13.8% [7.8–23.0] in Guadeloupe, 17.2% [9.5–29.2] in Martinique, 22.6% [14.4–33.5] in French Guiana, and 24.0% [16.3–33.2] on the island of Réunion. In mainland France, vaccination coverage is higher among 18-year-old girls (48.7% [40.6–56.9], adjusted prevalence ratio (APR) = 1.30 [1.04–1.62], p=0.022) compared to 15-year-olds (39.0% [32.4–46.0]), and among older girls (46.8% [42.9–50.8], APR=1.34 [1.08-1.67], p=0.008) vs. those who were not (31.8% [25.2-39.2]), when parents had the highest incomes (2nd and 3rd income tertiles by consumption unit: 53.8% [47.2–60.2] and 56.4% [50.1–62.6]) vs. 1st tertile: 32.9% [28.0–38.2], respectively RPa=1.35 [1.10–1.66], p=0.004 and RPa=1.24 [0.99–1.55], p=0.063), consider themselves financially comfortable compared to those in a less favorable financial situation: "comfortable": 58.8% [50.9–66.2] vs. "barely making ends meet": 25.6% [15.5–39.3], RPa=0.56 [0.35-0.88], p=0.013 and “cannot make ends meet without debt”: 14.5% [5.7-32.2], RPa=0.35 [0.14-0.85], p = 0.02), have at least 5 years of education beyond high school (59.2% [52.1–66.1], RPa=1.32 [1.06–1.65], p=0.012) compared to those without a diploma or with a diploma below the high school level (36.7% [30.5–43.4]), and those of French nationality by birth (46.3% [42.5–50.0]) compared to those who acquired French nationality (24.1% [14.9–36.4], RPa=0.59 [0.38–0.91], p=0.017). Conclusion—Despite an improvement in HPV vaccination coverage among girls in France, coverage remains moderate, particularly among economically disadvantaged populations. These results call for strengthening vaccination promotion efforts, particularly among these populations, with a view to reducing health inequalities.
