2001–2017 Overview of External Reports of Healthcare-Associated Infections. Proportion of reports involving multidrug-resistant bacteria, highly resistant/emerging bacteria, or Clostridium difficile
The Healthcare-Associated Infection Reporting System (SIN) is an early warning and response system that has been mandatory for all healthcare facilities since 2001. It has contributed to the detection and monitoring of several outbreaks. This article describes the SINs received from 2001 to 2017: number, trends, type of HC, epidemiological characteristics, microorganisms, and sites of infection involved. A focus was placed on SINs involving: i) a multidrug-resistant bacterium (MDRB): methicillin-resistant Staphylococcus aureus (MRSA), third-generation cephalosporin-resistant Enterobacteriaceae (3gC-REB), primarily due to extended-spectrum beta-lactamase (ESBL) production, carbapenem-resistant Acinetobacter baumannii (CRAB) and carbapenem-resistant or multidrug-resistant Pseudomonas aeruginosa (PARI), ii/ a highly resistant bacterium (HReB): carbapenemase-producing Enterobacteriaceae (CPE) and glycopeptide-resistant Enterococcus faecium (GREF) or iii/ Clostridium difficile (CD). During the study period, 23,012 SINs (100,658 patients) were received. The most frequently identified microorganisms were: Enterobacteriaceae (31%, of which 8% were EC3GR and 81% were EPC), Enterococcus spp. (9%), Acinetobacter baumannii (6%), and Staphylococcus aureus (8%). In total, 46.1% of SINs involved MRSA (13.3%), MREB (25.0%), MRSA and MREB (0.7%), or CD (7.1%). This proportion accounted for the bulk of the increase in the number of SINs, rising from 2.5% in 2001 to 66% in 2016, and from 0% to 54.3% for SINs involving a BHRe. The sharp increase in BMR, BHRe, and C. difficile SINs reflects the heightened awareness among hospital hygiene teams regarding their control. These efforts must be continued. To distinguish SINs related to antibiotic resistance and provide a more appropriate description of these events, a dedicated BHRe tool has been available since September 2017 within the e-SIN application.
