PEPS'PE Study: Prioritization of Health Effects to Be Monitored Under Santé publique France’s Endocrine Disruptor Surveillance Program - Methodological Report
Santé publique France plans to expand its monitoring of health indicators related to endocrine disruptors (EDs) as part of the National Strategy on Endocrine Disruptors (SNPE), the National Health and Environment Plan, and WHO recommendations, but also in response to growing expectations from public authorities and civil society regarding the agency’s work on EDs. For this reason, Santé publique France has begun prioritizing health effects linked to EDs, with the aim of structuring its monitoring program on this topic. Santé publique France proposes a method for prioritizing health effects to be monitored due to their suspected link to exposure to EDs. This method is based on ranking effects first according to the strength of existing evidence, and second according to the epidemiological and societal importance of implementing this surveillance. To assess these two criteria, the agency proposes a complementary method combining both available literature data and the opinions of experts and stakeholders in the field via the Delphi consultation method. This work is part of a public health monitoring framework and not a substance- or exposure-based approach: the aim is to track the evolution of a health indicator in the general population due to its suspected link to exposure to PE, not to characterize the health effect of a PE product (or group of products). The Delphi method used facilitates the organization of a consultation with a group of experts to obtain a final, consensus-based opinion from the group. It will be structured around two online questionnaires: one addressing the scientific aspects and one addressing the societal aspects. To respond to these questionnaires, two expert groups were formed, bringing together experts from the scientific field as well as stakeholders in the field of PE. With the aim of establishing the agency’s surveillance program, Santé publique France will then analyze the feasibility of implementing surveillance for the effects identified as priorities following the consultation. The proposed methodology aims to find the best balance between scientific robustness, operational feasibility, and clarity for all. This report provides a summary of the methodology proposed by Santé publique France to prioritize health indicators in the context of exposure to environmental pollutants.
