Survival of Cancer Patients in Metropolitan France, 1989–2013. A Study Based on Cancer Registries from the Francim Network. Part 1: Solid Tumors

The objective of this study is to provide, using population data from the Francim network registries, updated estimates of observed and net survival at 1, 3, 5, and 10 years following a cancer diagnosis, and to present trends in survival over time. Estimates of 15-year survival are also being produced for the first time in France. The study included all new cases of solid cancers diagnosed between 1989 and 2010 in individuals aged 15 years or older in 19 metropolitan departments, representing 502,063 incident cases. Results are presented for 37 cancer sites, across all stages. Net survival among individuals with solid tumors varies considerably by cancer site over the 2005–2010 period. Cancers with a poor prognosis (5-year net survival rate below 33%) account for 31% of solid cancers in men and 17% in women. Conversely, cancers with a good prognosis (5-year net survival rate of 66% or higher) account for 57% of solid cancers in women and 44% in men. For the same type of cancer, women most often have a higher survival rate than men. Survival decreases with age for all solid cancers. An improvement in net survival is observed for most solid cancers over the 1989–2010 period, though this improvement is less pronounced among older patients. This improvement may, in part, be attributed to therapeutic advances as well as earlier diagnosis, leading to better management but also to a diagnostic lead due to screening. Depending on the cancers studied, these two explanatory factors may combine. Other factors may also contribute to explaining survival trends and are discussed for each cancer site studied.
