Investigation and Management of a Scabies Outbreak in a Nursing Home. Pyrénées-Atlantiques, November 2005
In late 2005, a nursing home in the Pyrénées-Atlantiques region reported several cases of scabies affecting residents and staff to the Ddass. Alongside the implementation of preventive measures, an epidemiological investigation was conducted to identify potential risk factors and monitor the progression of the outbreak. The effectiveness of this management approach was also evaluated. A cohort study was conducted among all residents (n=107) and staff (n=65). A confirmed case of scabies was defined as the occurrence, after September 15, 2005, of scabies confirmed by a physician. Case surveillance continued until January 15, 2006. This study confirmed the existence of 24 confirmed cases of scabies (incidence rate = 14%), occurring over a period of approximately 2 months. The incidence rate among residents (10.3%) was half that of staff (25.5%). For staff, the occupation of nursing assistant (RR=5.9; p=0.04) and use of a specific locker room (RR=5.9; p=0.02) were risk factors. Among residents, wheelchair use (RR=4.4; p=0.03) and participation in certain activities were significantly associated with the disease. A crisis response team, coordinated by the hygiene department and bringing together expertise in decision-making, care, and logistics, was established. A mass treatment program for the entire community was organized involving a single oral dose of ivermectin followed by a second dose on day 8. Ultimately, this response required significant resources. Although the first case was reported too late, the various recommendations were effectively implemented within the facility. Contact tracing among external contacts proved more difficult. Treatment with Ivermectin, due to its one-time administration, greatly facilitated the management of the outbreak, and no further cases were reported.
