A Study of Cryptorchidism and Hypospadias Cases Treated Surgically in France from 1998 to 2008 in Boys Under 7 Years of Age, Based on Data from the Medical Information Systems Program (PMSI)
Cryptorchidism and hypospadias are urogenital malformations whose increasing prevalence has been reported in some countries and which are suspected to be linked to exposure to endocrine disruptors. Data from the national hospital admissions database (PMSI) were analyzed for the period 1998–2008 in France to track trends in surgical procedures for cryptorchidism and hypospadias in boys under 7 years of age. Results: for cryptorchidism, the average age at surgery is 3 years. The rate of surgical procedures in mainland France is 2.51 per 1,000 boys per year and increases annually by 1.8%. In the overseas departments, this rate is significantly lower: 1.42 per 1,000 boys per year, but it is increasing by 4% per year. Regional variations show the highest incidence rates relative to the national average in Auvergne, Brittany, Champagne-Ardenne, Franche-Comté, Languedoc-Roussillon, Lorraine, and Nord-Pas-de-Calais. Regarding hypospadias, the average age at the time of surgery is 1.7 years. The rate of surgical procedures in mainland France is 1.10 per 1,000 boys per year, and is increasing annually by 1.2%. Regional variations show that incidence rates are higher in Languedoc-Roussillon, Provence-Alpes-Côte d'Azur, Alsace, and Rhône-Alpes. These regional variations are not yet explained. The increase in surgical intervention rates, while potentially indicative of a rise in disease incidence, may also reflect changes in medical practices or PMSI coding over time. (R.A.)
