Experiences of Violence and Health Status: Findings and Public Health Implications from Nursing Assessments Conducted in the Île-de-France Region Among Migrant Populations (October 2015–July 2018)
Introduction: Since the fall of 2015, more than 14,000 newly arrived migrants housed in the Île-de-France region have received nursing assessments. Conducted by teams from Samusocial de Paris and the French Red Cross, these assessments provide information on both health status and exposure to violence. Materials and Population: The study includes health assessments conducted between October 2015 and July 2018 by mobile teams in emergency shelters housing migrants evacuated from encampments, then at the Initial Reception Center (CPA), at the Emergency Shelter for Migrant Women and Families in Ivry-sur-Seine (CHUM d’Ivry), and in two Reception and Situation Assessment Centers (CAES). The surveyed population is 93% male, with an average age of 27. As nursing assessments have evolved, we leverage this variability to generate results and draw public health lessons. Results: mobile assessments did not explicitly ask about experiences of violence; recoding of open-ended responses allows us to calculate a frequency of 7% (n=327) of victims among all patients. With the introduction of explicit, pre-coded questions (CPA-CHUM d’Ivry-CAES), this same frequency rises to 27%, and to 80% when limited to those with whom the question was addressed. Experiencing violence is closely correlated with sociodemographic profile (age, family/marital status, country of origin), type of migration journey, and physical and mental health status. Discussion and conclusion: addressing violence experienced by migrants appears essential in the healthcare context. To this end, staff should be trained, and interview conditions should be established to enable screening and appropriate care following nursing assessments.
