Establishment of an epidemiological surveillance system for trauma-related mortality among children under 15 in mainland France

In mainland France, injuries were the leading cause of death among children under 15 in 2009: 459 deaths (source: Center for Epidemiology on Medical Causes of Death (CépiDc)), of which 236 were due to accidents in daily life (AcVC). These raw data do not provide detailed insight into the circumstances of the trauma that led to death. The feasibility study "Mortality from Accidents of Daily Life Among Children Under 15 (MAC-15)," launched in 2009 in Île-de-France, Nord-Pas-de-Calais, and Provence-Alpes-Côte d'Azur, aimed to understand precisely how these accidents occurred and to identify their risk factors. Secondary objectives were to test feasibility and analyze the potential for expanding or generalizing surveillance of these deaths. Case identification was based on death certificates, contacts with regional health agencies, and media monitoring. A detailed questionnaire was completed by the certifying physician. In 2009, the MAC-15 survey recorded 76 deaths from accidental injuries among children under 15 years of age in the three regions. The victims were more often boys. The most common cause of death was drowning, followed by choking, falls, and fires. The survey effectively made it possible to precisely determine the risk factors for fatal accidents among children and, in most cases, to identify the cause of the fatal accident. The feasibility conclusions of the survey are positive; the data collection method is effective and minimally burdensome, and the quality and completeness of the data are good. The establishment of a routine, long-term survey—"Detailed Analysis of Traumatic Deaths in Children Under 15 (ADT)"—is proposed as a follow-up to MAC-15, expanded to cover all fatal childhood injuries in metropolitan France.
