Epidemiology of Ischemic Heart Disease in France

Introduction – Cardiovascular diseases are the leading cause of death worldwide. Ischemic heart disease (IHD), including acute coronary syndromes (ACS) with or without ST-segment elevation and chronic coronary syndromes, is one of the main causes. The objective of this article was to describe the epidemiology of IHD in France in 2022. Methods – Adults hospitalized for IHD in 2022 were identified in the National Health Data System (SNDS). Patient characteristics, hospital care, and one-year prognosis were described. The prevalence of PCI as of January 1, 2023, was estimated by combining prior hospitalizations and long-term illness (ALD) benefits. PCI-related mortality was estimated based on death certificates from 2021. Results – In 2022, 242,227 adults were hospitalized for a CPI in France (452 per 100,000 person-years). The prevalence of CPIs was estimated at 2.98 million cases (5.6% of the adult population). In 2021, 31,391 people died from a PCI, representing 4.8% of all deaths. The average age at hospitalization for a PCI was 69.3 years, and 29.0% of patients were women. Exposure to cardiovascular risk factors was high. The average length of hospital stay was 4.9 days; 55.9% of patients underwent angioplasty with stent placement, and 4.5% underwent coronary artery bypass grafting. The in-hospital mortality rate was 3.6%. Six months after the initial hospitalization, 22.0% of patients had been admitted to a rehabilitation unit (42.9% for ACS with ST-segment elevation). During the year following initial hospitalization, 84.5% of patients had received antiplatelet agents, 82.6% a statin, 68.8% a beta-blocker, and 67.7% a renin-angiotensin-aldosterone system inhibitor. One year after the initial hospitalization, the rehospitalization rate for an ACS was 29.9% and the all-cause mortality rate was 9.3%. Conclusion – The burden of PCI remains high in France. These results suggest that it is necessary to maintain and improve primary and secondary prevention of PCI in order to improve the prognosis and quality of life of the 3 million coronary patients.
