A cross-sectional study of physicians' diagnostic and treatment practices for scabies: low adherence to guidelines may explain the outbreak

Background: The ongoing increase in the incidence of scabies in France highlights the need to investigate the reasons behind the spread of this epidemic. Objective: The aim of this study is to examine the management practices of general practitioners, pediatricians, and dermatologists regarding scabies cases, specifically in terms of diagnosis, prescribing preventive and curative treatments, and clinical follow-up. Methods: A cross-sectional study among private practitioners in Brittany was conducted in April 2013. The sample was stratified by type of physician (dermatologists, general practitioners, and pediatricians), by urban or rural area, and by department (the four departments in the region). Factors likely to explain physicians’ adherence to national recommendations for the treatment and management of scabies cases were analyzed using multivariate logistic regression. Results: The study estimates the incidence of scabies in Brittany at approximately 12,200 cases over the seven months preceding the study (September 2012–March 2013). Only 34% (95% CI [27.8%, 40.4%]) of physicians followed the treatment duration recommendations, and 53.2% (95% CI [46.7%, 59.5%]) treated close contacts of a diagnosed case. Dermatologists were the only group, independent of other factors, to predominantly follow all treatment recommendations. Being a general practitioner (OR = 0.049, 95% CI [0.01, 0.22]) or a pediatrician (OR = 0.056, 95% CI [0.01, 0.2]) was significantly associated with non-compliance compared to dermatologists. Conclusion: The scabies epidemic is becoming a major public health problem in France. The majority of practitioners treating cases do not appear to follow national best practice guidelines, which could explain the continued spread of the parasite. However, the study shows that most physicians are seeking additional information for themselves and their patients. (R.A.)
