Is the improvement in healthcare utilization among adults with diabetes observed in France between 2001 and 2007 similar across all socioeconomic levels? Entred Studies
Objective: To examine changes in social inequalities in healthcare utilization among people with diabetes between 2001 and 2007. Materials and methods: The addresses of 9,868 and 6,204 adults with diabetes, randomly selected from the health insurance databases for the years 2001 and 2007, were geolocated at the IRIS level (neighborhoods of approximately 2,000 inhabitants). Changes in annual RAS were analyzed according to a deprivation index divided into quintiles, from the least deprived (DEF-, Q1) to the most deprived (DEF+, Q5), after adjusting for age, sex, and antidiabetic treatment. Results: The proportion of women among the DEF+ group was higher (2007, Q5: 50% vs. Q1: 43%), and the DEF+ group was younger (62 vs. 64 years). Between 2001 and 2007, insulin therapy became more common among DEF+ patients (2001, Q1: 20%, Q5: 20%; 2007, Q1: 21%, Q5: 27%, p = 0.040), as was enrollment in the ALD program (2001, Q1: 75%, Q5: 78%; 2007, Q1: 82%, Q5: 87%, p = 0.378). While frequent visits (≥ 12/year) to a general practitioner had decreased less among DEF+ patients, while remaining more frequent (2001, Q1: 29%, Q5: 36%; 2007, Q1: 21%, Q5: 34%, p = 0.017), endocrinology visits had increased slightly among DEF+ (2001, Q1: 10%, Q5: 7%; 2007, Q1: 10%, Q5: 9%, p = 0.612). Consultations with an ophthalmologist every 2 years had increased slightly among DEF- patients (2001, Q1: 68%, Q5: 59%, 2007, Q1: 71% vs. Q5: 59%, p = 0.385); visits to the dentist every two years remained less frequent among DEF+ individuals, with no change (Q1: 53%, Q5: 48%, p = 0.913). Regarding laboratory tests, no differences were observed based on the deprivation index for the three HbA1c measurements (2001, Q1–Q5: 30%; 2007, Q1: 39%, Q5: 38%, p = 0.700) or for lipid profiles, albuminuria, and serum creatinine. Conclusion: While the impact of social inequalities on healthcare utilization remains significant, the gap between levels of deprivation appears stable or even narrowing, except for ophthalmology visits.
