Organized Breast Cancer Screening - Epidemiological Assessment. 2005 Data

The nationwide rollout of the national breast cancer screening program, in accordance with French guidelines, has been in effect since 2004, with the exception of French Guiana, where the program began in mid-May 2005. For 2005, data from 98 out of 99 departments were analyzed, representing 1,871,497 women screened through the organized screening program. The participation rate for 2005 was 44.8%, compared to 40.2% in 2004. Primary radiologists deemed 10.1% of mammograms abnormal, and 42.5% of these were still deemed abnormal after follow-up, representing 4.3% of screened women. This immediate follow-up thus cleared 57.5% of suspicious images and avoided the need for a subsequent recall. In cases where abnormalities were confirmed, it allowed for immediate management. Second-reader radiologists reviewed 95.2% of all mammograms and recalled 1.6% of women for abnormalities. A follow-up mammogram was ordered for 3.6% of screened women (placed under surveillance). Cytological or histological examinations were performed on 1.1% of them. Among the 0.7% of women who underwent a surgical biopsy, 79.5% were diagnosed with cancer. In total, 12,413 cancers were detected, compared to 10,640 during the same period in 2004, representing a cancer rate of 6.7 per thousand, which is the same as in 2004. The report highlights improved compliance of screening procedures with national guidelines, as well as progress in the program’s activity and quality indicators. These indicators are generally in line with European recommendations, though significant disparities remain across the country, including a first-read positive mammography rate exceeding 15% in 13 departments and biopsy rates ranging from 0.07% to 1.58% depending on the department. The quality and standardization of the data submitted for national evaluation are improving, but the completeness of certain variables remains insufficient in some departments. (R.A.)
