Full coverage of breast cancer screening, both within and outside the organized screening program, from 2016 to 2022 in mainland France
Introduction – Participation in the National Organized Breast Cancer Screening Program (PNDOCS) is approximately 50%. This places France far below the European recommendation of 70% participation. However, there is a high level of breast cancer screening outside the PNDOCS. The objective of this study is to estimate the total coverage rate for breast cancer screening in mainland France from 2016 to 2022. Methods – The data used come from the Regional Cancer Screening Coordination Centers (CRCDC), mammography procedures reimbursed by the national health insurance system included in the National Health Data System (SNDS), population estimates from the National Institute of Statistics and Economic Studies (INSEE), and SNDS disease mapping based on breast cancer treatments, to estimate the number of women ineligible for screening. Results – Total coverage is estimated at 60% in mainland France in 2021–2022, a slight decrease over the period (it was 62% in 2016–2017), with a marked decline in 2019–2020. The share of screenings outside the PNDOCS program among all screenings is estimated at 18% in 2021–2022; it was 17% in 2016–2017. Discussion – These results indicate that in France, women’s use of breast cancer screening is similar to that in other European countries, but that many undergo screening outside the PNDOCS program. However, screening is more effective when conducted as part of the organized program, particularly because it includes a second reading of mammograms, higher quality throughout the screening process, and an evaluation of the results.
