A Study of Hospital Morbidity Related to Alcoholic Liver Disease Based on Data from the Medical Information Systems Program (PMSI), France 2008

Introduction. Few data are available on the health consequences associated with alcohol consumption. Among these consequences, alcohol-related liver diseases are the most common. The objective of this study is to analyze hospital morbidity associated with these conditions. Methods. Two selection algorithms were developed using the 2008 national PMSI database: the first (A1) corresponds to hospital stays listing, as a primary, associated, or related diagnosis, an alcohol-related liver disease or ALD (ICD-10 code: K70 or K74.6 if associated with F10); the second (A2) selects, based on this algorithm, hospital stays directly related to ALDs or one of their complications (closed list of 50 codes). For each algorithm, after linking hospital stays, a descriptive analysis of the patients was conducted. Results. In 2008, 64,390 patients were hospitalized with an ALD (A1), of whom 37,284 were hospitalized in connection with one of these diseases or one of its complications (A2). The standardized rates of hospitalized patients are higher among men than among women (158/100,000 and 48/100,000 person-years (PY) for A1 and 91/100,000 and 28/100,000 PY for A2) and vary by geographic region (higher in the North/Northwest and lower in the South). The mortality rate is 13% (n = 8,700 for A1; n = 5,000 for A2). Discussion and conclusion. The standardized rate of hospitalized patients for A1 yields a hospital morbidity indicator for MAFs that is lower than the prevalence in the general population, due to the selection bias inherent in the PMSI hospital database. The A2 algorithm, for its part, allows for the determination of the minimum number of deaths attributable to alcohol-related diseases. Analysis of trends over the 2005–2009 period should make it possible to observe the consequences of changes in alcohol consumption (increases among women and young people). (R.A.)
