HIV and Bacterial STIs in Bourgogne-Franche-Comté. 2023 Report.

HIV Infections and AIDS

  • HIV Surveillance:

    • Satisfactory participation of laboratories in Bourgogne-Franche-Comté (BFC) in the HIV serology reporting survey (LaboVIH)

    • Regional completeness of reporting has been below the national level since 2020; the proportion of complete reports (clinician and laboratory technician) remains low in 2023 (55%).

  • HIV Testing:

    • Activity (regardless of service availability) is increasing, accompanied by a slight rise in the positivity rate. The 2023 positivity rate is the highest in BFC since 2014; nevertheless, the region remains among those with the lowest detection rates

    • Regional and departmental screening rates below the national level

  • HIV diagnosis: nearly half (46%) of HIV infections are diagnosed at a late stage. The proportion of people aged 50 and older in the observed figure in France has been at a higher level since 2016

  • HIV incidence (number of new infections in the region): Regional estimate of 105 cases in 2023

  • AIDS diagnosis: downward trend. Trend to be confirmed due to low completeness of data. In 2023, 64% were unaware of their HIV-positive status prior to developing AIDS.

Chlamydia trachomatis (Ct) infections

  • Screening: regional rate on the rise; nevertheless among the lowest in mainland France

  • Diagnosis: regional rate among the lowest in mainland France, with disparities between departments. Rate in Doubs higher than the national rate excluding the Île-de-France region (74.1 vs. 70.9 per 100,000 inhabitants)

  • The increase in diagnoses was greater than that in screenings between 2022 and 2023 among men under 50

  • Chlamydia trachomatis infection is the most common STI in the region and is the one most frequently treated at the region’s CeGIDDs

Gonococcal infections

  • Screening: regional rate on the rise; nevertheless among the lowest in France

  • Diagnosis: regional rate among the lowest in France, with disparities between departments. Rates in Doubs (23.8) and the Territoire de Belfort (25.4) are within the national average excluding the Île-de-France region (24.3 per 1,000 inhabitants)

  • Increase in diagnoses greater than that in screenings among men under 50

Syphilis

  • Screening: regional rate on the rise; nevertheless among the lowest in France

  • Diagnosis: regional rate among the lowest in France. Rates in Haute-Saône (5.6) and the Territoire de Belfort (5.8) are close to the national rate excluding the Île-de-France region (6 per 1,000 inhabitants)

  • Increase in syphilis screening and cases between 2022 and 2023 among men aged 26–49 (+6 percentage points)

Prevention and Action

  • Rollout of the national campaign focused on combined HIV and STI prevention

  • Launch of a symbolic regional initiative focused on supporting people who engage in chemsex, launched in 2024 and continuing into 2025
