Rapid HIV screening and diagnostic tests at CDAGs: impact on the reporting of other serology results and the value of implementing follow-up measures for patients who have been lost to follow-up
Rapid HIV screening and diagnostic tests (RHDTs) are recommended for HIV testing in community-based settings. Their use in CDAG/CDDIST centers may increase demand but could affect the turnaround time for results of other tests performed concurrently, as these results are delivered later. The objectives of this study were: i) to measure the impact of introducing RDTs on the reporting of results for other screened infections, and ii) to measure the impact of implementing telephone follow-up measures (sending text messages or making calls) for patients on the number of unreported results following an HIV RDT. This study began in April 2011 and was conducted over 15 months, in two successive phases, at the CDAG/CDDIST centers in Orléans and Blois. During the first phase, rapid diagnostic tests (RDTs) and ELISA tests were offered every other week. A total of 445 individuals were included (197 rapid tests; 248 ELISA tests). The loss-to-follow-up (LTFU) rate for HIV result delivery was significantly lower among patients who received a rapid test (0%) than among those who received conventional HIV serology (7.3%) (p<0.001), whereas the LOR rate for the delivery of results for tests other than HIV was significantly higher among patients who received a rapid test (19.8% versus 13.2%; p=0.05). During the second phase, a direct phone call to the patient, in cases of non-return following a rapid test, reduced the non-return rate for other serological tests to 10.2%. The use of rapid tests in CDAG/CDDIST centers promotes access to screening. However, when non-rapid screening tests must be performed concurrently, measures to follow up on lost-to-follow-up patients must be implemented. (R.A.)
