Prevention, Screening, and Management of Hypertension in France: The Perspective of General Practitioners, France, 2019
Introduction - Hypertension is a common chronic condition that constitutes a major cardiovascular risk factor. Its diagnosis and management are primarily carried out by general practitioners (GPs). In this context, the objective of this study was to describe the opinions and perceptions of French GPs regarding hypertension. Methods - A survey was conducted among a panel of GPs affiliated with the College of General Medicine using an electronic questionnaire. Analyses were adjusted for the physician’s age and sex according to the French distribution of GPs (data from the Shared Directory of Healthcare Professionals, RPPS 2018). Results - A total of 753 GPs responded to the questionnaire and were included in the analyses; 83% of GPs reported being strongly involved in the prevention of hypertension. Self-measured blood pressure (SMBP) and ambulatory blood pressure monitoring (ABPM) are widely used for the diagnosis of hypertension. In this survey, 96% of physicians recommended lifestyle modifications as first-line treatment when managing hypertensive patients, with 52% combining this with drug therapy. However, 93% of physicians reported that their patients expressed reservations about the prescription of antihypertensive medications. The main reservations about treatment and barriers to treatment adherence highlighted by GPs concerned difficulties in getting hypertensive patients to accept and understand the disease, especially in the absence of symptoms. Conclusion - This study highlighted that, according to general practitioners, the lack of time during consultations was a barrier to preventive care, and that patients’ poor knowledge and understanding of the disease remained limitations to treatment adherence and the control of hypertension in the French population.
