Duration of breastfeeding in France (Épifane 2012–2013)
Objectives: Using data from the Épifane study on infant feeding during the first year of life, the duration of breastfeeding, the degree of exclusivity, and breastfeeding rates at 3, 6, and 12 months were estimated for the first time at the national level in France. Methods: The study included a random sample of children born in the first quarter of 2012 in 136 randomly selected maternity wards in mainland France. Data on infant feeding were collected through interviews at the maternity ward and at 1, 4, 8, and 12 months. The World Health Organization’s definitions of exclusive and predominant BF were used. Kaplan-Meier curves were used to describe the progression of breastfeeding between birth and the first birthday, taking into account lost-to-follow-up cases. Results: Among the 3,365 children included in the analyses, 2,806 (83%) were followed up to 12 months. At 3 months, 39% of the children were still being breastfed: 10% exclusively, 11% predominantly, and 18% also receiving commercial infant formula (IF). At 6 months, only one in four children was still being breastfed, and more than half of them were receiving IF as a supplement. At one year, only 9% of children were still receiving breast milk. Among children who were breastfed at birth (74%), half were still being breastfed at 15 weeks, with the median duration of exclusive or predominant breastfeeding estimated at 3.5 weeks. Conclusion: The results of this study show that, in light of current recommendations, mothers who have chosen to breastfeed should be encouraged to continue breastfeeding for as long as possible, up to 6 months. Further analysis will help identify factors associated with the duration of breastfeeding and thus aid in targeting interventions to better align with these recommendations. (R.A.)
