Investigation of a case of tuberculosis among healthcare workers, France, 2004–2007.
Introduction - Given the low incidence of tuberculosis in France, tuberculosis control efforts must focus in particular on limiting nosocomial transmission of the disease. Healthcare facilities house particularly vulnerable populations, and when a healthcare worker contracts infectious tuberculosis, this requires the implementation of investigations—which can sometimes be burdensome—to identify contacts who need to be screened. Method - A retrospective descriptive study based on nosocomial infection reports (SIN) involving healthcare facility staff and received by the French Institute for Public Health Surveillance (InVS) between 2004 and 2007. Results - Twenty-six SINs for tuberculosis involving healthcare facility staff were received by the InVS during the study period. All SINs involved healthcare workers. Twenty-three of the 26 were cases of potentially contagious tuberculosis. None of the cases involved drug-resistant strains. Investigations conducted to identify secondary cases involved 1,224 healthcare professionals and 791 patients. Only one case of active tuberculosis was identified. Discussion - The yield of investigations into tuberculosis cases among healthcare facility staff is very low and may warrant a review of certain recommendations regarding the scope of these investigations.
