Surveillance of Outbreaks of Acute Respiratory Infections and Acute Gastroenteritis in Nursing Homes for the Elderly in the Pays de la Loire Region, 2010–2013
A surveillance system for clusters of acute respiratory infections (ARI) and acute gastroenteritis (AGE) occurring in nursing homes (Ehpad) was established in the Pays de la Loire region in 2010. The first two surveillance seasons helped improve operational surveillance and management tools in the region, based on two evaluations conducted in 2011 and 2012 among nursing homes and partner organizations. The results of the third surveillance season (2012–2013) confirmed that the system had been successfully adopted by the nursing homes and had a positive impact on the practices implemented to manage these outbreaks. Between 2010 and 2013, 824 clusters of cases, accounting for nearly 15,000 cases, were reported: 401 clusters of acute respiratory infections (ARI) and 423 clusters of gastrointestinal infections (GI). Nearly two-thirds of the region’s nursing homes submitted at least one report during the three seasons. The completeness rate of reports was estimated at 81% for both ARI and GEA. The analysis identified three types of complementary indicators relating to the intrinsic characteristics of outbreaks, reporting practices, and outbreak management in nursing homes. Areas for further methodological work on these data were proposed (analysis of epidemic curves, estimation of influenza vaccine efficacy, and investigation of suspected foodborne rather than person-to-person transmission in GEA episodes).
